Employee Group Clients
Your benefit administrator or human resource department will provide the enrollment code.
Individual Clients
Your enrollment code is provided in your Welcome letter.
Western Life Assurance
The enrollment code is the client’s policy number, which begins with EE, EW, WF, WFM, or WG.
Ivari
The enrollment code is the client’s policy number, which begins with 8, 77, or 99.
Specialty Life
The enrollment code is “SLI” plus the client’s policy number.
IA American/American Amicable
The enrollment code is “IA” plus the client’s policy number after removing the two leading zeroes.
Mutual of Omaha
The enrollment code is the client’s policy number, which begins with UR.
Transamerica
The enrollment code is the client’s policy number.
AIG/Careington
The enrollment code is the client's policy ID, which begins with C.
